Overview

Comprehensive Case Coordination includes:

1. Assessment + Evaluation: comprehensive review to identify client/family strengths and needs and make clinical recommendations. Includes comprehensive treatment planning.
2. Crisis Intervention: Available 24/7 to provide immediate clinical care, with the goals of preventing condition deterioration, ensuring safety, and delivering treatment in the least restrictive setting.
3. Family and individual support services: Structured services to ensure the safety and well-being of children, support family preservation, or enhance reunification. These needs-based services include skill-building, behavioral management, home and community visits, safety planning, and crisis management. Includes monthly progress reporting.
4. Parent/caregiver training: providing both informal and formal support and education, including training, and access to psychoeducational and support groups.
